Membership Benefits

Functional Medicine Membership:

  • Affordable Access: Low monthly fee with no insurance billing. You control your pricing and access to professional medical services and other amenities.
  • Insurance for Labs/Radiology: Use your insurance plan for labs or radiology ordering when it makes sense or you can choose to pay cash and receive discounted rates on lab services.

Medicare Recipient Members:

  • Added Benefits: Join as Functional Medicine Members.
  • Opt-Out Billing: Shirley Olson, NP-C, IFMCP, is credentialed by Medicare but has chosen to “opt out,” meaning clinic visits won’t be billed to Medicare.
  • Lab/Radiology Orders & Referrals: Medicare will cover lab/radiology orders and referrals to specialists/physical therapists.
  • Virtual Services: Basic Membership $40/month or Membership Plus $45/month Virtual Visits are $100/30 minutes.
 

Ideal for:

  • Adults 18 and Over: Easy, direct communication with your Health Care Professional.
  • Root Cause Focus: Healthcare Professional addresses the root cause of illness, not just symptoms.
  • Proactive Health: For those who want to actively improve and maintain their health.

Urgent Care:

  • Life-Threatening Emergencies: Call 911 immediately. We do not offer urgent or emergent care.

Virtual Visits Away from Home:

  • Accessible Anywhere: Virtual visits are available wherever you are.
  • In-Person Evaluation: We do not provide in-person evaluations, However, if needed, we’ll recommend appropriate nearby care and follow-up with you virtually when you return home.

Membership Pricing and Options

Pricing:

  • Basic Membership: $40/month
  • Membership Plus: $45/month
  • Virtual Visits: $100/half hour

Discounts:

  • Children (17-23, attending school) qualify for discounts.

Payment Options:

  • No long-term contracts required.
  • Payment can be made annually, quarterly, or monthly.

Membership Information

Patient Limit:

  • We limit our practice size to devote more time and energy to each patient. Once the ideal size is reached, a waiting list will be established.

Cancelling Membership:

  • You can cancel your membership with 30 days’ notice. Annual memberships will be refunded at a prorated rate.
