Tier 2 – Maintenance Membership: $90/month (18+)

• Annual Payment: $900/year (two months free; $75/month)
• Couples Rate: $150/month (includes domestic partnerships)
• Adult Child Rate: $20/month (adult children 17-23)

• 6 visits/year

• Annual Payment: $490/year (two months free; $42/month)

Functional Medicine Member BHRT Pelleting Fee:

Non-Member BHRT Pelleting Fee:
• Women: $400
• Men: $700

***Individuals who are maintained on prescription medications that require more frequent monitoring will need to choose Tier 1 or 2 membership levels or for tier 3 add ala carte visits up to 2 times/year.

Virtual Clinic Visits: Self-schedule or request via Spruce app text, clinic call, or patient portal messaging. Can be scheduled several months in advance. Availability may be affected by clinician vacation or sabbatical.
Administrative Assistance: Available Monday – Thursday, 8:30 am – 3:00 pm via Spruce app text, call, or patient portal messaging.
Office Closures: Announced in advance due to holidays.
