Tier 1 – Vital Force – Advanced, holistic care for optimal energy and longevity: $120/month (18+)

• Annual Payment: $1200/year (two months free; $100/month)
• Couples Rate: $200/month (includes domestic partnerships)Adult Child Rate: $30/month (adult children 17-23*)

• Additional ala carte visits $60/30 min

Tier 2 – Centered Balance – Intermediate support for maintaining equilibrium: $90/month (18+)

• Annual Payment: $900/year (two months free; $75/month)
• Couples Rate: $150/month (includes domestic partnerships)
• Adult Child Rate: $20/month (adult children 17-23)

Includes:
• 6 visits/year
• Additional ala carte visits $60/30 min
• Prescription management, specialist referrals, lab Analysis, hormone and thyroid balancing

Tier 3 – Root Energy – Basic wellness guidance to maintain core health: $50/month (18+)

• Annual Payment: $490/year (two months free; $42/month)
• Couples Rate: $84/month (includes domestic partnerships)
• Adult Child Rate: $10/month (adult children 17-23)
***Adult children 17-23 attending high school/college/university

Includes:
• 2 visits/year
• Additional a la carte visits $60/30 min
• Medical weight loss medication management, prescription management, specialist referrals, hormone and thyroid balancing (*these services require a minimum of 2 a la carte visits per year)
• Portal and Text messaging with Timely Responses

***Individuals who are maintained on prescription medications that require more frequent monitoring can choose Vital Force or Centered Balance membership levels or for Root Energy add a la carte visits a minimum of 2 times/year

Administrative Assistance: Available Monday – Thursday, 8:30 am – 3:00 pm via Spruce app text, call, or patient portal messaging. Office is closed Thursday 3:00 pm – Monday 8:30 am. Requests during this time will be addressed during open office hours.

Scheduled Office Closures: In observance of Christmas, New Year, Thanksgiving, Independence Day, and Labor Day. We appreciate your understanding.
